What information do I need to bring with me when making funeral service arrangements?

Many families are facing a multitude of decisions when a love one passes. In an attempt to assist you with the arrangements for your love one, the information below serves as a guide.

Information to fill out the death certificate:

  • Decease full legal name
  • Occupation (if retired, what they did most of their life)
  • Date of birth
  • Place of birth
  • Social Security Number
  • Address of Residency
  • Spouse name (Maiden name if female)
  • Father's name
  • Mother's maiden name
  • Place of disposition
  • Discharge papers if veteran
  • Educational background

You need to: (we can assist you with the below listed items if you need our help)

  • Contact you minister and establish time and place of service. Also find out if a musician is available to you.
  • Decide what cemetery you would like to use.
  • For a veteran, have or get the DD214 for burial in order to obtain a flag and to bury in a national cemetery. (National Cemeteries are not open on weekends)

We will provide you with:

  • The proper form and information to place the obituary in the newspaper.
  • A general price list to assist you in purchasing services, casket and any other merchandise.
  • An itemized contract and discuss payment arrangements.

Note: We ask that you bring the clothing doing the arrangement or the day after the casket selection. Clothing includes everything except shoes. If desiring a wig, please present with clothes. If a beautician is a desire, we can provide one or you may provide you own.

How much does a funeral service cost?

  • Funeral costs depends on what you would like.

Is embalming required?

  • Except in certain special cases, embalming is not required by law. Embalming may be necessary however, if you select certain funeral arrangements such as a funeral service with public viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as Direct Cremation or Immediate Burial.

What are the basic services of the funeral director?

  • The funeral home must be available 24 hours a day to receive calls.
  • Execute the proper papers according to state law as it pertains to permission to embalm.
  • Execute any special request of the family.
  • Obtain necessary information to activate removal of remains with proper authorized permission.
  • Dispatch first call persons and vehicle to properly remove the body from the designated location. Serenity Funerals & Cremations can coordinate transporting a body form anywhere in the world. This is coordinated by the funeral home, and must comply with FTC and OSHA Rules.
  • The funeral home arranges embalming, once they have permission.
  • The funeral home sets up time for the family to come in to make arrangements, which are preformed by a licensed Director.
  • The Director must comply with FTC rule by providing the family with a General Price List of Goods and Services.
  • The Funeral Director obtains Vital Statistic information for the Death Certificate.
  • The Funeral Director arranges for the Death Certificate to be taken to doctor for cause of death. Then picked-up and taken to Health Unit where the number of certified Death Certificates ordered by family will be obtained for the family.
  • The Funeral Director/Funeral Home coordernates the time and place of service, newspaper notice, ordering of a vault if needed , ordering the casket spray, ordering a police escort, arranging limousines for pickup and filing all necessary forms (Insurance, Permits, Death Certificates, VA, Social Security, etc.)
